A secret, unauthorized list delayed psychotherapy care for 87 veterans at one Veterans Affairs hospital.

The delays at the VA hospital in Omaha, Nebraska were associated with "training deficiencies," according to the Omaha World Herald.

No employees have been fired in connection with the list, but one resigned.

The list violated rules set up after a 2014 scandal that showed VA employees delaying appointments and falsifying paperwork.
